• Alteplase / stroke calculator: this is one of our highest-priority updates. The current page contains older three-hour-window and contraindication language. We plan to rebuild it around the 2026 AHA/ASA guideline, current alteplase and tenecteplase pathways, expanded thrombectomy eligibility, imaging, blood-pressure management and pediatric considerations.
• NIH Stroke Scale / acute stroke tools: GlobalRPh currently lacks a dedicated NIHSS tool. We plan to evaluate adding one with appropriate training and use limitations, along with selected acute stroke scores that genuinely add clinical value.
• Phenytoin Dosing Calculator: we plan to modernize loading and maintenance guidance, free-phenytoin measurement, albumin correction, renal dysfunction, interacting medications, enteral feeding, nonlinear kinetics, fosphenytoin and current therapeutic-drug-monitoring references.
• Anticonvulsants: we plan to update currently marketed antiseizure medications, indications, pregnancy/teratogenicity considerations, renal/hepatic dosing, drug interactions, status-epilepticus roles and newer precision-epilepsy therapies.
• Benzodiazepine Converter: we plan to strengthen the distinction between approximate equivalence and direct switching, clarify half-life/active-metabolite differences and improve tapering and CNS-depressant safety language.
• Alzheimer's Disease drug table: this page predates the current anti-amyloid era. We plan to add lecanemab and donanemab, amyloid confirmation, APOE/ARIA context, MRI monitoring, infusion/injection considerations, antithrombotic issues and current biomarker-guided diagnostic care.
• Multiple Sclerosis: this is a major modernization project. The current page still begins with daclizumab/Zinbryta-era content and does not reflect the current breadth of disease-modifying therapy. We plan to rebuild the table around current DMT classes, monitoring, pregnancy, vaccination, infection/PML risk, escalation and phenotype/activity-based treatment.
• Parkinson Disease: we plan to update adjunctive dopaminergic therapy, newer agents, device-assisted treatment, motor fluctuations, dyskinesia, hallucinations, orthostatic hypotension, sleep, cognition and advanced-therapy referral.
• ALS Agents: we plan to update currently available disease-modifying and gene-targeted therapies, oral versus IV edaravone, mutation-specific treatment, respiratory/nutrition support and the status of agents that have entered or left the market.
• Migraine: we plan to rebuild the older triptan-centered table around current acute and preventive therapy, CGRP-targeted agents, gepants, ditans where applicable, medication-overuse headache, pregnancy, cardiovascular constraints and vestibular migraine crossover.
• Restless Legs Syndrome: we plan to update iron assessment, augmentation risk, alpha-2-delta ligands, dopamine agonists, refractory disease and medication triggers.
• Muscle Relaxants: we plan to separate spasticity, acute musculoskeletal spasm and neurologic indications, with stronger CNS-burden, fall-risk and geriatric safety context.
• Myasthenia gravis: we plan to develop a dedicated GlobalRPh page covering symptomatic treatment, conventional immunotherapy, FcRn inhibitors, complement inhibitors, crisis recognition and medications that can worsen neuromuscular transmission.
• CNS medication burden: we plan to connect the updated CNS burden tool more directly with Neurology, Geriatrics, falls, cognition and deprescribing rather than leaving it isolated in the calculator archive.
• Neurologic drug toxicity: we plan to add a stronger medication-induced neurology pathway for cefepime neurotoxicity, serotonin toxicity, anticholinergic burden, drug-induced movement disorders, seizure-threshold lowering medications and renal-dose accumulation.
