Clinical background

Eye disease ranges from chronic pressure control to vision-threatening emergencies

Ophthalmology includes disorders of the ocular surface, cornea, lens, optic nerve, retina, retinal vasculature, extraocular structures and visual pathways. Some conditions evolve slowly, such as primary open-angle glaucoma or cataract, while others—including acute angle closure, infectious keratitis, retinal vascular occlusion, giant cell arteritis and sudden visual loss—can require urgent specialty evaluation.

The previous GlobalRPh page already has an unusually strong glaucoma-drop library. This rebuild preserves that strength while broadening the portal to dry eye and blepharitis, ocular infection, retinal disease, diabetic retinopathy, AMD, cataract, myopia and systemic disease with ophthalmic manifestations. Medication tables remain prominent, but current AAO and FDA resources now provide a clearer verification layer while older individual pages are updated.

Visual acuity & symptoms Pain, photophobia, sudden loss, field defects, flashes/floaters and diplopia can change urgency.
IOP & optic nerve Glaucoma management depends on more than one pressure reading; structural and functional progression matter.
Ocular surface Tear-film, lid, meibomian-gland and preservative effects can influence symptoms and adherence.
Retina & systemic health Diabetes, hypertension, vascular disease, inflammation and neurodegeneration can manifest in the eye.

Four focused tools already built around glaucoma and dry-eye pharmacotherapy

These are the most distinctive resources on the current portal and remain prominent in the new design. The update roadmap below identifies where the underlying product lists and evidence need to be brought forward.

These tools are medication-selection aids rather than diagnostic instruments. Glaucoma target pressure, structural/visual-field progression, angle anatomy, ocular-surface disease, contraindications, adherence, laser/surgical options and specialist examination remain central to treatment decisions.

Glaucoma & ocular hypertension

GlobalRPh's deepest ophthalmic pharmacology collection

The existing portal already separates the major pressure-lowering drug classes. The rebuild keeps those class pages together and adds current AAO guidance for open-angle, glaucoma-suspect and angle-closure disease.

Lower IOP Choose therapy by efficacy, mechanism, contraindications, tolerability, adherence and ocular-surface burden.
Reassess target Target IOP is dynamic and should respond to structural and visual-field progression.
Protect ocular surface Preservative exposure and drop burden can affect symptoms, adherence and quality of life.
Escalate when needed Laser and surgical strategies are part of modern glaucoma care when drops are insufficient or unsuitable.

The GlobalRPh glaucoma tools rely heavily on older 2000s–2019 studies and a 2016 comparative-effectiveness analysis. We are retaining the calculators while updating current product availability, newer glaucoma agents, target-pressure framing, laser/surgical context and 2025 AAO Preferred Practice Pattern guidance.

Dry eye, eyelid disease & ocular surface

Dry-eye therapy now extends well beyond artificial tears, Restasis and Xiidra

The portal now separates aqueous-deficient dry eye, evaporative/meibomian disease, preservative exposure and blepharitis so the ocular-surface workflow is easier to navigate.

Dry eye is heterogeneous. Tear-film instability, meibomian-gland dysfunction, inflammation, lid disease, medication/preservative exposure, systemic autoimmune disease and environmental factors may contribute in different proportions. Treatment therefore should not be reduced to ranking drops from “weakest” to “strongest.”

Ocular infection, cornea & inflammation

Antibacterials, antivirals, corticosteroids, fortified preparations and corneal emergencies

Pain, photophobia, reduced vision, corneal opacity/infiltrate, contact-lens-associated keratitis, trauma, chemical exposure or suspected intraocular infection should not be managed as uncomplicated “pink eye.” Fortified ophthalmic therapy also requires current sterile-compounding, concentration and institution- specific verification.

Retina, macula & diabetic eye disease

AMD, diabetic retinopathy, retinal vascular disease and intravitreal therapy

Retinal therapy is underrepresented on the older portal despite a large modern intravitreal-drug landscape. This section connects current AAO guidance with GlobalRPh drug monographs and retinal research.

Sudden monocular visual loss, new diplopia with neurologic findings, suspected giant cell arteritis, papilledema, acute painful red eye with vision loss, retinal-detachment symptoms or acute angle closure require urgent diagnostic evaluation rather than routine topical-medication selection.

GlobalRPh's glaucoma and ophthalmic-drug tables have served as practical medication references for years, while eye care has changed across glaucoma treatment, dry eye, blepharitis, myopia control, intravitreal therapy and ocular-surface safety. We plan to preserve the strongest tools while updating product availability, evidence, current guidance and clinical workflow.

Pages we plan to refresh

  • Glaucoma Drug Selection Tool: this is one of our highest-priority ophthalmology updates. We plan to reconcile its 2000s–2019 reference base with the 2025 AAO glaucoma PPPs, verify current products and formulations, incorporate newer agents such as omidenepag where appropriate, and improve laser/surgical escalation context.
  • Potency of Glaucoma Drops: the current ranking relies primarily on a 2016 network meta-analysis plus older package-insert data. We plan to refresh comparative IOP reduction, combination products, dosing burden, adherence, ocular-surface tolerability, preservative exposure and the limitations of cross-trial potency comparisons.
  • BAK-Free / Preservative-Free Tool: we plan to verify every currently marketed preservative-free and BAK-free formulation, remove unavailable products, distinguish truly preservative-free from alternative-preservative products, and add administration/device considerations.
  • Glaucoma Drug-Class Tables: prostaglandins, beta blockers, alpha agonists, carbonic-anhydrase inhibitors, ROCK inhibitors and fixed combinations will be rechecked for current formulations, generic availability, contraindications, systemic adverse effects and current labeling.
  • Dry Eye Topical Options: the current app only offers Restasis, Xiidra and Lacrisert as prescription choices. We plan to add current therapies including perfluorohexyloctane/Miebo and cyclosporine 0.1%/Vevye, verify other currently marketed products, and separate evaporative, aqueous-deficient and inflammatory dry-eye phenotypes.
  • Blepharitis: GlobalRPh does not currently have a dedicated modern blepharitis page. We plan to build one covering anterior/posterior blepharitis, meibomian-gland dysfunction, Demodex, lid hygiene and FDA-approved lotilaner/Xdemvy.
  • Ophthalmic Antibacterials: the existing table is extensive but needs current product verification, stronger stewardship language, clearer distinction between conjunctivitis and keratitis, contact-lens risk, culture indications and avoidance of inappropriate antibiotic-steroid combinations.
  • Fortified Ophthalmic Preparations: we plan to modernize concentrations, preparation/stability references, sterility and compounding safeguards, storage, beyond-use considerations, and the role of fortified antibiotics in severe corneal infection.
  • Ophthalmic Antivirals: we plan to update currently marketed agents, HSV/VZV disease distinctions, epithelial versus stromal keratitis context, systemic-antiviral crossover and ophthalmology follow-up requirements.
  • Ophthalmic Corticosteroids: we plan to verify current formulations and newer agents, update post-operative/inflammatory indications, and strengthen warnings around IOP elevation, infection, delayed healing and inappropriate unsupervised use.
  • Ophthalmic NSAIDs: we plan to update current perioperative products, cataract-surgery use, pain/inflammation indications and corneal-safety precautions.
  • Mydriatics / Cycloplegics: we plan to update current formulations, pediatric dosing/safety, anticholinergic systemic effects, angle-closure precautions and the modern role of low-dose atropine in myopia management.
  • Ocular Anti-Allergy / Decongestants: we plan to distinguish true allergy treatment from cosmetic redness relief, verify current OTC/Rx products, address rebound hyperemia and avoid implying that vasoconstrictor whitening drops treat underlying disease.
  • Retina / intravitreal therapy: we plan to build a dedicated GlobalRPh retinal-drug hub covering aflibercept/Eylea HD, ranibizumab, faricimab, geographic-atrophy therapies and current indications, while linking treatment choice to AAO retina guidance rather than presenting an unstructured drug list.
  • Diabetic retinopathy: we plan to connect Endocrinology and Ophthalmology with screening, pregnancy, glycemic-change considerations, DME and intravitreal therapy using the current AAO PPP.
  • AMD / geographic atrophy: we plan to create a dedicated page separating non-neovascular AMD, neovascular AMD and geographic atrophy, with current imaging, supplementation and intravitreal-treatment context.
  • Pediatric myopia: we plan to update the older atropine article into a modern myopia-control hub incorporating optical approaches and the 2025 FDA-authorized myopia-control spectacle-lens option.
  • Urgent visual-loss pathway: we plan to add a clinician-facing triage page covering giant cell arteritis, retinal detachment, retinal vascular occlusion, acute angle closure, optic neuritis, infectious keratitis, orbital disease and neurologic causes.
