Renal dosing, restrictions and evidence limits

At CrCl 50 to <80, initiate 156 mg on day 1 and 117 mg on day 8, both deltoid. The usual subsequent monthly dose is 78 mg, deltoid or gluteal. Depending on response and tolerability, renal maintenance choices are 39, 78, 117 or 156 mg; the maximum is 156 mg monthly. CrCl <50 is not recommended. Establish oral paliperidone or risperidone tolerability first when the patient has never received either.
