Renal dosing, restrictions and evidence limits

At Cockcroft-Gault CrCl 50 to <80 mL/min, give 234 mg on Day 1 in the deltoid, then 78 mg monthly in the deltoid or gluteal muscle. Maintenance may be adjusted to 39, 78, 117 or 156 mg; the renal maximum is 156 mg monthly. Use is not recommended below CrCl 50. This is not the SUSTENNA two-injection initiation schedule; no Day-8 ERZOFRI dose is added by this application. Switching and missed-dose instructions require their own full product protocol.
