Renal dosing, restrictions and evidence limits

At CrCl 50 to under 80 mL/min, start 3 mg once daily; the maximum is 6 mg daily after response/tolerability assessment. At CrCl 10 to under 50, start 1.5 mg once daily, with a maximum of 3 mg daily after reassessment. Use below 10 is not recommended because it was not studied. Swallow ER tablets intact. These oral instructions must not be transferred to long-acting injectable paliperidone.

Oral extended-release tablets

Adult schizophrenia or schizoaffective disorder: initially 6 mg/day; range 3-12 mg/day; maximum 12 mg/day. Adolescent schizophrenia: start 3 mg/day. Weight below 51 kg: range 3-6 mg/day, maximum 6; weight at least 51 kg: range 3-12, maximum 12.

Monthly injection - INVEGA SUSTENNA only

IM only. Establish tolerability with oral paliperidone or oral risperidone when naive to oral paliperidone or oral/injectable risperidone. Initiate 234 mg deltoid on day 1, then 156 mg deltoid 1 week later. Recommended monthly maintenance 117 mg, with available alternatives 39, 78, 156 or 234 mg depending on response; maintenance may be deltoid or gluteal.

Deltoid needle: weight at least 90 kg, 1.5-inch 22G; below 90 kg, 1-inch 23G. Gluteal: 1.5-inch 22G at any weight. The source refers missed initiation/maintenance doses to the complete prescribing information and does not provide a missed-dose algorithm locally.
