HAFYERA: general renal and geriatric sections must both be read

The geriatric renal restriction is more restrictive than the general adult mild-impairment pathway. This distinction is present in both the RX mirror and official label, not merely one rendering.

Before applying: Do not silently override the age-specific statement. Seek product-label/pharmacy or manufacturer clarification before applying a general adult pathway to an elderly patient with renal impairment.

Also check: HAFYERA: general renal and geriatric sections must both be read

Renal dosing, restrictions and evidence limits

Sections 2.4 and 8.6 allow the specified prior-product adjustment and stabilization pathway for CrCl 50 to <80, but not use at CrCl <50. Section 8.5 separately states that HAFYERA is not recommended in elderly patients with mild, moderate or severe renal impairment. Both statements occur in this official label; this is not merely a mirror discrepancy. Age-specific eligibility requires clarification rather than automatic application of the general adult pathway.

Renal dosing, restrictions and evidence limits

General renal guidance permits CrCl 50 to <80 only after adjustment and stabilization on the one-month product, with the specified transition pathway; CrCl <50 is not recommended. Separately, the geriatric section says use is not recommended in elderly patients with mild, moderate or severe renal impairment. The age-specific restriction is retained, not overridden by the general section.
