Clinical interpretation | Version 6.8

Find the difference.
Keep the context.

Search documented disagreements and product cautions across the local library. Compare complete source versions on the drug page before choosing a regimen.

A difference is not always a contradiction

Compare indication, formulation, route, population, renal method, dialysis conditions and source date. A safety update or source-quality failure is not an alternative dose. These notes are not independent clinical approval, and sources are not ranked or averaged.

Migalastat oral capsules (Galafold)

Before use: Migalastat oral capsules (Galafold)

Use indexed eGFR and the qualifying genetic indication; this is not a daily or pediatric regimen.

What to reconcile

Confirm exact indication, indexed renal estimate and alternate-day schedule; use the newer primary label for full prescribing context.

source ambiguity

Miglustat (ZAVESCA)

The printed renal bands overlap at exactly 50

The selected source uses an indexed creatinine-clearance unit and overlapping renal ranges.

What to reconcile

Do not silently convert units or choose an unsupported boundary. Confirm the current product label and clarify the exact threshold.

ambiguous threshold

Miglustat (ZAVESCA)

Exactly 50 occurs in two printed renal bands

The source assigns different starting frequencies to ranges that share 50 as a boundary, and prints clearance normalized to 1.73 m2.

What to reconcile

Confirm the exact product label and boundary interpretation before prescribing. The tool does not resolve this by rounding, averaging or converting an indexed measure silently.

formulation scope

Miglustat - OPFOLDA with POMBILITI

OPFOLDA: paired therapy and every-other-week renal dosing

The OPFOLDA instruction belongs to a weight-based regimen paired with POMBILITI, not a daily Gaucher-disease miglustat schedule.

What to reconcile

Confirm paired therapy, body weight, renal method and infusion timing. Do not transfer a ZAVESCA regimen.

institutional regimen

Minocycline

IV label daily ceiling versus institutional loading and high-dose practice

The MINOCIN IV label sets a renal-impairment ceiling of 200 mg per 24 hours. UCSF lists a 200-mg loading dose followed by 100 mg every 12 hours without renal adjustment, and selected higher-dose treatment with specialist input. Initial-day exposure or high-dose maintenance can exceed the label ceiling.

What to reconcile

This is a real exposure difference, not just alternate wording. Obtain infectious-disease/antimicrobial-stewardship input and document the exact IV product, indication, renal status, total first-day exposure and monitoring before choosing an approach. Do not silently merge the label cap with an institutional schedule.

guideline difference

Minocycline - MINOCIN injection

Minocycline injection: label ceiling versus institutional dosing

The injection label limits total daily dosing in renal impairment. UCSF describes a loading dose and selected higher-dose practice without a renal reduction. A loading day or high-dose regimen is not silently reconciled with that ceiling.

What to reconcile

Keep the label and institutional pathways separate; obtain stewardship adjudication and retain BUN, creatinine and magnesium-vehicle monitoring.

evidence limit

Mitapivat - PYRUKYND tablets

PYRUKYND: limited PK data are not a dosing algorithm

Only the stated higher-function band has the cited exposure comparison; lower-function data are limited or absent.

What to reconcile

Display the evidence gap rather than a broad no-adjustment conclusion.

retrieval gap and route

Mitomycin pyelocalyceal solution (JELMYTO)

DailyMed recovered renal guidance; the RX page remains incomplete

The fetched RX rendering stopped before usable renal instructions. A separate exact JELMYTO DailyMed summary now supplies the renal restriction and obstruction precautions.

What to reconcile

Use the labeled pyelocalyceal formulation, not IV mitomycin or another urinary formulation. An alternative-label import does not convert the truncated RX page into a successful RX renal review.

population scope

Mixed amphetamine salts - MYDAYIS

MYDAYIS: adult and adolescent severe-renal ceilings differ

The adolescent severe-impairment instruction is more restrictive than the adult ceiling. The ESRD restriction is separate.

What to reconcile

Confirm age, exact product and indexed renal measure; do not use the adult maximum for an adolescent.

source scope and renal guidance

Molnupiravir - LAGEVRIO capsules

Before use: Molnupiravir - LAGEVRIO capsules

What to reconcile

evidence limit

Momelotinib - OJJAARA tablets

OJJAARA: low-eGFR PK observations do not establish dialysis dosing

The studied lower renal boundary is unusual; ESRD requiring dialysis remains unknown.

What to reconcile

Preserve the actual population boundary and avoid creating a dialysis regimen.

pk prediction not validation

Motixafortide - APHEXDA subcutaneous injection

A prediction about severe impairment is not observed evidence

The mirror predicts little severe-renal effect but explicitly states that it was not evaluated.

What to reconcile

Keep this classified as PK-only; do not translate an expectation into a proven no-adjustment or dialysis regimen.

population scope

Mycophenolate mofetil - MYHIBBIN oral suspension

MYHIBBIN: kidney-transplant dose ceiling is not every transplant setting

The selected severe-impairment ceiling is stated for kidney-transplant recipients. Mycophenolic-acid products have separate formulations and evidence.

What to reconcile

Preserve transplant organ, formulation and monitoring context; do not declare all mycophenolate products interchangeable.

evidence limit

Mycophenolic acid - MYFORTIC delayed release

MYFORTIC: pharmacokinetics are not a universal renal dose

The renal discussion draws on mycophenolate-mofetil data and metabolite exposure; it does not supply a universal renal dose or interchangeable product ceiling.

What to reconcile

Use the complete transplant-specific regimen and monitoring rather than inferring a no-adjustment protocol from this PK supplement.

boundary units

Nadolol - CORGARD tablets

CORGARD: indexed clearance and interval ranges

The table prints BSA-indexed CrCl and interval ranges. Replacing these with unindexed clearance or a midpoint changes the source instructions.

What to reconcile

Retain the printed units and bands; resolve fractional boundary gaps without inventing an interval.

evidence limit

Naldemedine - SYMPROIC tablets

SYMPROIC: renal pharmacokinetics are not a class-wide regimen

Similar single-dose exposure and lack of dialysis removal do not establish renal instructions for other opioid antagonists.

What to reconcile

Keep the adult indication, product, interaction review and PK-study scope attached to this card.
