EMTRIVA versus NIH: explicit renal schedule deviation

The standalone capsule/solution label reduces dosing at selected bands where NIH uses no adjustment. NIH footnote b explains its incorporation of some fixed-dose-product guidance.

Before applying: Choose and document one applicable institutional/specialist approach. Do not average schedules or silently relabel the NIH approach as the standalone product label.

Product / population
Adult standalone EMTRIVA: capsules and 10 mg/mL oral solution have distinct renal schedules. Not automatic guidance for FTC-containing combination tablets.

Renal dosing, restrictions and evidence limits

>=50200 mg every 24 hours240 mg (24 mL) daily
30 to 49200 mg every 48 hours120 mg (12 mL) daily
15 to 29200 mg every 72 hours80 mg (8 mL) daily
<15 or hemodialysis200 mg every 96 hours60 mg (6 mL) daily

On hemodialysis days, administer after dialysis. Efficacy of these reduced-renal-function schedules has not been clinically evaluated; pediatric renal data are insufficient. NIH guidance differs at selected bands and is stored separately.

Renal dosing, restrictions and evidence limits

NIH lists the usual adult FTC dose as a 200 mg capsule daily or 240 mg (24 mL) oral solution daily. It uses no adjustment at CrCl 30 to 49 mL/min and in hemodialysis, with administration after dialysis. Footnote b explicitly incorporates dosing from certain FTC-containing fixed-dose products despite the standalone EMTRIVA label's reductions. Keep this expert-guidance approach separate from the label schedule; it is not a silent label correction or universal pediatric rule.
