For adult HIV treatment, the 200/300 mg tablet is given every 24 hours at CrCl >=50 mL/min and every 48 hours at 30 to 49. It is not recommended below 30, including hemodialysis. For PrEP, use is not recommended below CrCl 60; do not apply the treatment-only every-48-hour schedule to prevention. Clinical efficacy of the renal interval adjustment has not been evaluated, and pediatric renal dosing data are insufficient.
