Streptomycin

Usual Dosing (Adults)

Tuberculosis:
Daily therapy: 15 mg/kg/day IM (max: 1 g)
Directly observed therapy (DOT)- twice weekly: 25-30 mg/kg IM (max: 1.5 g)
Directly observed therapy (DOT)- 3 times/week: 25-30 mg/kg IM (max: 1 g)

Streptococcal endocarditis: 1 g q12h (IM/IV) x 7d, then 500 mg q12h x 1 week.

Renal Dosing

[>50 ml/min]: q24h.

[CRCL 10-50 ml/min]: Give q24-72 hours.

[<10 ]: Give q72-96 hours.

Hemodialysis

Dose as for CrCl<10, on dialysis days give dose after dialysis
