Seizure disorders: Initial: 125-250 mg/day orally at bedtime. Increase by 125 to 250 mg/day q3-7 days. Usual dose: 750-1500 mg/day in divided doses 3-4 times/day. Maximum dosage: 2 grams/day.
Renal Dosing
[CRCL 50-80 ml/min]: give q8h
[10-50]: Give q 8-12 hours.
[ <10 ]: Give q12-24 hours.
Hemodialysis
Dose as for CrCl<10 - on dialysis days supplement with 1/3 of usual dose AD
