According to the CDC, Coronaviruses are a large family of viruses that can cause illness in both humans and animals.  Some variants infect only animals, however, in the case of COVID-19, it eventually spread from animals to humans and finally from human to human after further mutations.

     The most common symptoms of COVID-19 include fever, cough, sore throat, shortness of breath, and fatigue while other symptoms include headache, diarrhea, lymphopenia, and dyspnea. Identifying & Assessing 2019 Novel Coronavirus Patients


In the past 14 days since the onset of symptoms, check all that apply:


  Travel to China

  Has been in close contact with a person known to have COVID-19

  Patient lives in or has recently been in an area with ongoing spread of COVID-19



Does  the person have symptoms of lower respiratory illness.

Symptoms usually appear 2-14 days after exposure.

  Fever

  Cough

  Shortness of breath

  Difficulty breathing

  Persistent pain or pressure in the chest

  New confusion or inability to arouse

  Bluish lips or face

  Any other symptoms that are severe or concerning.


Please review the following:

(Check all that apply)

65 years and older   

Resident of long-term care facility or nursing home

  Suffers from chronic lung disease or moderate to severe asthma

Suffers from a serious heart condition (heart failure, angina, cardiomyopathy, other)

  Compromised immune system: (primary immunodeficiency disease, poorly controlled HIV, transplant patient, cancer, prolonged use of corticosteroids, other.)

Severe obesity (body mass index [BMI] >40)

  Underlying medical conditions such as diabetes, renal failure, or liver disease.
