Renal dosing, restrictions and evidence limits

For PAH, the source starts patients with CrCl 51-80 or 31-50 mL/min at 20 mg once daily, with an increase to 40 mg once daily according to tolerability. Severe impairment is an avoidance setting; the dosage section prints CrCl <30 mL/min and hemodialysis, and the renal-use section advises avoidance in severe impairment. Dialysis does not meaningfully correct exposure.
