Renal dosing, restrictions and evidence limits

Section 2.2 advises considering 10 mg (2 mL) for heart-failure initiation at eGFR 30-50 mL/min/1.73 m2. That renal sentence does not give a distinct administration frequency. CAROSPIR is not therapeutically equivalent to ALDACTONE. Check potassium within one week of starting or titrating, then regularly; closer monitoring may be needed with renal impairment or potassium-raising medicines. Monitor volume status and renal function. Do not silently convert the tablet regimen or invent a dialysis dose.
