Renal dosing, restrictions and evidence limits
For CrCl below 60 mL/min, chronic non-cancer pain: oral tablets 150 mg each morning, or injection 6 mg subcutaneously once daily. For advanced illness: subcutaneous dosing every other day as needed is 0.075 mg/kg below 38 kg; 4 mg at 38 to less than 62 kg; 6 mg at 62 to 114 kg; or 0.075 mg/kg above 114 kg. These advanced-illness doses are not the chronic non-cancer pain schedule. Use the vial when a prefilled syringe does not match the prescribed dose.
