Renal dosing, restrictions and evidence limits

The adult opioid-induced-constipation dosage is 0.2 mg orally once daily. In a single-dose study, exposure was similar in mild, moderate and severe renal impairment and ESRD requiring hemodialysis. Renal strata used indexed eGFR; the normal comparison group was described by estimated CrCl. Similar pre- and post-hemodialysis concentrations indicated that hemodialysis did not remove naldemedine. This is a general-dose plus pharmacokinetic summary, not a separately validated dialysis regimen or a class-wide rule for other opioid antagonists.
