Renal dosing, restrictions and evidence limits

DDAVP injection is contraindicated below Cockcroft-Gault CrCl 50 mL/min. Ensure normal sodium before initiation or resumption. For treatment monitoring, reassess sodium within 1 week, at about 1 month and periodically; increase frequency for older patients and other high-risk groups. Minimize fluid intake from 1 hour before until 8 hours after administration. Loop diuretics, systemic/inhaled glucocorticoids, hyponatremia/history, SIADH, polydipsia, relevant acute illness, heart failure and uncontrolled hypertension carry contraindications. Monitor blood pressure during administration. The PK study is not a renal-dose ladder or a dialysis regimen.

Renal dosing, restrictions and evidence limits

The mirror contraindicates DDAVP injection at CrCl below 50 mL/min. Hyponatremia/history, SIADH, polydipsia, loop diuretics, systemic or inhaled glucocorticoids, fluid/electrolyte-disturbing illness, heart failure and uncontrolled hypertension have separate contraindication statements. Impaired-renal PK findings do not supply a reduced-dose exception. These restrictions concern the named injection and labeled uses; other desmopressin products and specialist off-label uses require their own review.
