Usual Dosing (Adults)

15-25mg/kg q24h (maximum 2500mg/day)

Renal Dosing

[CRCL >50] No change.

[10 – 50 ]: Normal dose q24-36h

[<10 ]: Normal dose q48h.

Hemodialysis

Normal dose q48h. On dialysis days, schedule dose after dialysis.
