Pancreatic abscess, infected psuedocyst

Piperacillin-tazobactam 3.375 grams every 6 hours OR

Ciprofloxacin 400mg IV q8h, or Levofloxacin 500mg IV q24h, or moxifloxacin 400mg IV q24h)
PLUS
Metronidazole 500mg IV q6-8h.

Severe - >/= 30% of the pancreas is affected:

Imipenem 500mg IV every 6 hours OR
ertapenem 1gram IV q 24h, OR
doripenem 500mg IV every 8 hours OR
meropenem 1 gram IV q8h.
