Flagyl 500mg IV OR oral every 6 to 8 hours  
PLUS

[Ertapenem 1 gm IVPB q24h]
[Timentin  or  Piperacillin/tazobactam] +/-   Flagyl 500mg  IV every 6 to 8 hours  (Note Flagyl present for amoeba as well as anaerobes).
