Renal dosing, restrictions and evidence limits
For renal impairment, use serum creatinine in mg/dL, not a CrCl substitution. During monthly menstruation, for no more than 5 days: SCr >1.4 to 2.8, 1300 mg orally twice daily; >2.8 to 5.7, 1300 mg once daily; >5.7, 650 mg once daily. The source's otherwise usual menstrual regimen is 1300 mg three times daily for at most 5 days.
Keep the course limit with every renal band. This oral indication and its creatinine boundaries must not be transferred to IV products. Review the complete product contraindications, including thrombotic-risk and combined-hormonal-contraceptive restrictions, before prescribing.
