Renal dosing, restrictions and evidence limits
The renal table uses serum creatinine, not CrCl. SCr 1.36-2.83 mg/dL: 10 mg/kg IV twice daily. SCr 2.83-5.66: 10 mg/kg IV daily. Above 5.66: 10 mg/kg IV every 48 hours OR 5 mg/kg IV every 24 hours. The table prints 2.83 in both adjacent bands; this tool does not silently assign that exact boundary.
Reduction applies to doses before and after tooth extraction. Keep the labeled short course (2-8 days after extraction) and actual-body-weight context. IV only: neuraxial administration is contraindicated and can be fatal. No dialysis supplement or other-indication regimen is inferred.
