Renal dosing, restrictions and evidence limits

First determine the normal-function daily dose for the indication. The table maps 165/330/495/660 mg daily at CrCl >=60 to 82.5/165/247.5/330 mg daily in the printed CrCl 30-60 band, all once daily. The printed bands overlap at exactly 60; confirm the applicable label rather than silently choosing a row. LYRICA CR is not recommended below CrCl 30 or during hemodialysis; the label directs use of LYRICA instead. Immediate-release and CR milligrams are not interchangeable.
