Usual Dosing (Adults)

Diabetic peripheral neuropathy: start 50 mg orally three times daily; increase to 100 mg three times daily within 1 week depending on benefit/tolerability.

Postherpetic neuralgia: start 75 mg twice daily or 50 mg three times daily; may increase to 300 mg/day within 1 week. Usual maintenance 75-150 mg twice daily or 50-100 mg three times daily (150-300 mg/day). After 2-4 weeks at 300 mg/day with insufficient relief but good tolerability, may increase to 300 mg twice daily or 200 mg three times daily.

Renal Dosing

CrCl above 60 mL/min: no change. 30-60: 75-300 mg/day in 2-3 doses. 15-30: 25-150 mg/day once daily or in two doses. Below 15: 25-75 mg once daily. The indication-specific normal regimen must be established before selecting within a renal range.

Hemodialysis

Immediately after a 4-hour HD session, the source lists supplemental doses according to the daily regimen:

  • 25 mg daily: supplement 25 or 50 mg.
  • 25-50 mg daily: supplement 50 or 75 mg.

Renal guidance and important limits

LYRICA uses Cockcroft-Gault creatinine clearance and the indication-specific normal-renal total daily dose to select the corresponding renal-adjusted daily amount. Divide that total into the stated number of administrations; it is not milligrams per administration. A supplemental dose follows every 4-hour hemodialysis session in addition to renal-adjusted daily dosing. The printed categories overlap at 30 and 60 mL/min; do not conceal this source-table boundary issue. This immediate-release table is not a LYRICA CR conversion.
