Renal dosing, restrictions and evidence limits

For adult acute bacterial skin and skin structure infection, the ORBACTIV regimen is 1200 mg IV over 3 hours. Mild or moderate renal impairment requires no adjustment; severe impairment has not been evaluated. The PK analysis describes CrCl 50-79 and 30-49 mL/min groups. Oritavancin is not removed by hemodialysis, but no dedicated dialysis studies were conducted. ORBACTIV and KIMYRSA differ in preparation and infusion instructions; use the exact product, not a mixed regimen.
