CONTRAVE: a maximum does not replace titration

The qualitative renal ceiling belongs to a fixed combination. Parent-drug PK does not negate increased metabolite exposure.

Before applying: Do not invent numeric renal bands or apply this ceiling to naltrexone alone.

Also check: CONTRAVE: a maximum does not replace titration

Renal measure
Qualitative renal statement; no numeric renal algorithm inferred.

RX mirror: this is not independent corroboration of its underlying label. Check dated-label and formulation limitations.

Renal dosing, restrictions and evidence limits

For moderate or severe renal impairment, the maximum is two tablets per day: one each morning and evening. Each ER tablet contains naltrexone hydrochloride 8 mg and bupropion hydrochloride 90 mg. ESRD use is not recommended. This ceiling does not replace initial titration. The renal passage gives qualitative categories, not numerical clearance cutoffs. Although parent-drug pharmacokinetics were not significantly changed, metabolites had increased exposure. Do not transfer this combination schedule to either ingredient alone.
