Renal dosing, restrictions and evidence limits

Oral levocarnitine safety and efficacy have not been evaluated in renal insufficiency. Chronic high oral doses in severe impairment or dialysis-dependent ESRD can accumulate the potentially toxic metabolites TMA and TMAO.

A statement that levocarnitine can be removed from plasma by dialysis does not establish an oral dialysis regimen. Keep the injection's post-hemodialysis indication and schedule separate; do not use its existence to dismiss oral metabolite concerns.
