Review first - source-specific qualification

The post-HD regimen is indication-specific replacement therapy

The mirror uses dry body weight and trough-guided adjustment for injectable replacement.

Before applying: Confirm deficiency and the exact current injection label. Do not transfer this regimen to oral formulations.

RX mirror: this is not independent corroboration of its underlying label. Check dated-label and formulation limitations.

Selected sections only; not a complete prescribing monograph or independent clinician approval.

A hosted label mirror is not independent corroboration of its underlying label. Official confirmation remains pending unless a separate official card is shown.

Renal dosing, restrictions and evidence limits

For ESRD patients on hemodialysis, the mirror specifies a starting dose of 10-20 mg/kg of dry body weight after each dialysis session, injected into the venous return line over 2-3 minutes. Guide subsequent adjustment with pre-dialysis trough levocarnitine concentrations; a reduction to 5 mg/kg after dialysis may be considered as early as weeks 3-4.

This is indication-specific injectable replacement therapy, not a general renal reduction or an oral-equivalent dose. Confirm deficiency, concentration monitoring, tolerance and the dialysis schedule. The source is an April 2018 mirror; official-label currency remains unconfirmed in this pass.

What the dialysis evidence actually supports

Removal information alone does not define a maintenance dose, supplemental dose or dialysis schedule. A reported regimen still requires its product, indication, renal measure and dialysis assumptions to match the patient.
