Oral dosing remains 450 mg every 12 hours at indexed eGFR 15-89 mL/min/1.73 m2. Below 15, including hemodialysis, BAXDELA is not recommended. The IV severe-renal reduction is not an oral dose. Treatment duration depends on infection: 5-14 days for ABSSSI and 5-10 days for CABP.
