Renal dosing, restrictions and evidence limits
Use 300 mg IV every 12 hours at MDRD eGFR 30-89 and 200 mg every 12 hours at 15-29 mL/min/1.73 m2, over 60 minutes. Not recommended below 15, including hemodialysis. Monitor severe-renal patients for SBECD accumulation; consider oral therapy with rising creatinine and discontinue below 15. The table's older MDRD equation is source context, not a newly implemented calculation.
