Renal dosing, restrictions and evidence limits

Section 8.6 directs an initial dose based on renal impairment, careful monitoring for hypotension/bradycardia and cautious upward titration; pediatric renal pharmacokinetics remain unassessed. No numeric renal ladder is supplied. Because routine hemodialysis removes only a minimal amount, supplemental ONYDA XR after dialysis is unnecessary.

Measure heart rate and blood pressure before starting, after dose increases and periodically. The product is not interchangeable mg-for-mg with other clonidine formulations. Avoid abrupt withdrawal: the label requires gradual tapering to reduce rebound-hypertension risk. This is not a recommendation to use an unadjusted starting dose in renal impairment.

Pediatric renal PK has not been assessed. Base the initial dosage on the degree of renal impairment, monitor hypotension and bradycardia, and titrate cautiously. The selected renal passage gives no numerical starting schedule. Routine hemodialysis removes little clonidine, so a post-dialysis ONYDA XR supplement is not needed. This does not establish that the usual initial dose is appropriate in every renally impaired patient.
