Renal dosing, restrictions and evidence limits

No renal adjustment is required at CrCl 15 to below 90 mL/min, based on studies of oral cabotegravir. The effects in ESRD without dialysis are unknown. High protein binding makes dialysis unlikely to alter exposure; this does not establish a supplemental dialysis dose.

Confirm negative HIV-1 status before initiation and before every injection using testing appropriate for acute/primary infection. APRETUDE is prevention, not a complete treatment regimen for established HIV-1. Apply the full eligibility and missed-injection instructions separately.

Renal dosing, restrictions and evidence limits

The selected renal paragraph requires no adjustment at CrCl 15 to below 90 mL/min, based on oral cabotegravir studies. ESRD without dialysis has unknown PK. Dialysis is not expected to change exposure because protein binding is high; this is a pharmacologic expectation, not a dedicated dialysis dosing study.

Do not transfer the statement to cabotegravir/rilpivirine combination therapy. HIV-negative status and the product-specific testing requirements remain essential for PrEP.
