UNASYN label and UCSF do not use identical renal bands

At the printed value 30, the RX label table places patients in its at-least-30 row, whereas UCSF standard dosing places 30 in its 15-30 row. The RX heading also prints indexed clearance while UCSF uses mL/min.

Reconcile the renal metric and endpoint before selecting an interval. Do not choose whichever row is more convenient or merge the label dose range with an institutional interval.
