OSMOLEX ER: change dosing frequency and titration interval, not a daily fraction

The table uses MDRD indexed eGFR; other passages call the measure indexed CrCl. Reduced-frequency bands must not be converted into daily administration.

Before applying: Keep the two intervals distinct and reconcile source terminology; do not substitute a GOCOVRI schedule.

eGFR, mL/min/1.73 m2; retain body-surface-area indexing. Table specifies MDRD; other passages instead say indexed CrCl.

Renal dosing, restrictions and evidence limits

The renal table changes frequency and minimum titration interval, not the stated initial or maximum dose size. Table measure is MDRD eGFR (mL/min/1.73 m2):

60-89: one dose every 24 hours; increase no more often than weekly.

30-59: one dose every 48 hours; increase no more often than every 3 weeks.

15-29: one dose every 96 hours; increase no more often than every 4 weeks.

Below 15: contraindicated.

Other sections call the measure indexed creatinine clearance. This terminology discrepancy remains flagged. Do not interpret the phrase maximum daily dosage as permission for daily administration in the reduced-frequency bands.
