Renal dosing, restrictions and evidence limits

The adult renal table specifies 200 mg/day at CrCl 10-20 mL/min, 100 mg/day below 10, and on dialysis either 50 mg every 12 hours or 100 mg every 24 hours. These are IV cancer-therapy instructions, not a gout regimen. Monitor serum uric acid at least daily and individualize dose and frequency to keep it within the normal range.

Severe-renal and dialysis treatment has not been studied in pediatric patients; assess risks and benefits rather than transferring the adult table. Reconstitution, further dilution, infusion compatibility and the full treatment course must be checked in the original prescribing information.
