Renal dosing, restrictions and evidence limits
DYAZIDE is contraindicated in anuria, acute or chronic renal insufficiency, or significant renal impairment; the source does not define a numeric cutoff. Even mild impairment requires frequent, continuing electrolyte monitoring. Renal impairment increases hyperkalemia risk, especially with diabetes, older age or severe illness. Check potassium after initiation, dosage changes and illnesses affecting renal function.
Discontinue if increasing azotemia develops. Do not apply a single-agent hydrochlorothiazide statement to bypass the potassium-sparing combination restriction or invent an unprinted CrCl threshold.
