Renal dosing, restrictions and evidence limits

At creatinine clearance below 70 mL/min/1.73 m2 as printed, use one-half of the usual adult dose. The usual dose depends on indication and titration. Hemodialysis can rapidly remove topiramate; a supplemental dose may be needed based on session duration, dialysis clearance and residual renal clearance.
