No renal dosage adjustment is required at estimated CrCl of at least 15 mL/min, or below 15 in patients receiving chronic hemodialysis. On dialysis days, administer after the dialysis session. VEMLIDY is not recommended in end-stage renal disease without chronic hemodialysis. The source states that data are insufficient for renal dose recommendations in pediatric patients.
