Usual Dosing (Adults)

7.5 mg/kg IV every 8-12 hours. Infuse in D5W over 60 minutes; a pump/device may control the rate. Central access such as a PICC may reduce venous irritation. Vancomycin-resistant E. faecium: 7.5 mg/kg every 8 hours, duration based on site/severity. Complicated skin/skin-structure infection: 7.5 mg/kg every 12 hours for at least 7 days.
