Renal dosing, restrictions and evidence limits

The VOSEVI mirror recommends no dosage adjustment for any degree of renal impairment, including dialysis. This does not remove its separate restriction against use in moderate/severe hepatic impairment (Child-Pugh B or C). Regimen eligibility depends on prior HCV treatment and cirrhosis status.
