No renal adjustment is required at CrCl 50-80 or 30-49. With severe impairment below 30, consider a 25 mg starting dose because sildenafil exposure is increased. Usual Dosing (Adults)

Usually 50 mg as needed about 1 hour before sexual activity, permitted 30 minutes to 4 hours before. Adjust to benefit/tolerability from 25 to 100 mg, no more than once daily. The source calls for dose adjustment in older adults, renal impairment and with inhibitors identified by the original typo "CYP304A".
