Renal dosing, restrictions and evidence limits

For Parkinson disease, CrCl 30-50 mL/min needs no adjustment. With regular hemodialysis, start extended-release ropinirole at 2 mg once daily and titrate to response and tolerability; maximum 18 mg/day. No supplemental postdialysis dose is required. Severe impairment below 30 without regular dialysis has not been studied. These once-daily tablets are not the immediate-release RLS regimen. Do not transfer the IR starting frequency or treat an ER tablet as interchangeable without the label-specific conversion instructions.
