Original indications and adult dosing
Factor Xa inhibitor for nonvalvular atrial-fibrillation stroke/systemic-embolism prevention; DVT/PE treatment and recurrence prevention; and DVT prophylaxis after hip/knee replacement.
Take 15 and 20 mg tablets with food; 10 mg may be taken with or without food.
Nonvalvular AF: CrCl above 50 mL/min, 20 mg once daily with the evening meal; CrCl 15-50, 15 mg once daily with the evening meal.
Acute DVT/PE: 15 mg twice daily with food for 21 days, followed by 20 mg once daily with food. The historical source also gives 20 mg daily for long-term recurrence reduction.
Hip/knee-replacement prophylaxis: 10 mg once daily. The source lists 10, 15 and 20 mg tablets.
Renal Dosing and monitoring
Nonvalvular AF: avoid below CrCl 15; increased exposure is expected. Assess renal function periodically and more frequently when deterioration is possible. Discontinue if acute renal failure develops.
For DVT/PE treatment/recurrence prevention and hip/knee-replacement prophylaxis, this historical page says to avoid below CrCl 30 because of increased exposure/effect. For postoperative prophylaxis at CrCl 30-50, closely evaluate signs of blood loss. Discontinue for acute renal failure.
