Renal dosing, restrictions and evidence limits

Before UZEDY in renal impairment, titrate oral risperidone to at least 2 mg once daily. Based on response and tolerability after that titration, the recommended UZEDY regimen is 50 mg once monthly. UZEDY itself was not studied in renal impairment; the supporting oral-risperidone data show reduced combined clearance of risperidone and 9-hydroxyrisperidone. The source provides no numeric renal cutoff or dialysis-specific regimen. Do not substitute an every-two-month schedule, another long-acting formulation, or a different injection route. CYP2D6 inhibitors and CYP3A4 inducers need separate review.
