Renal dosing, restrictions and evidence limits

For CrCl 30 to 50 mL/min, initiate the extended-release product every other day. After one week, careful assessment of tolerability and response may allow daily administration. Subsequent increments are 0.375 mg, no more often than weekly, up to 2.25 mg once daily. No reduction is recommended above 50 mL/min. This ER product is not recommended below 30 mL/min or in hemodialysis. Use the full ER label to select the starting strength; do not substitute an immediate-release renal table.
