In adults with CrCl <30 mL/min, start at half the usual initial dose: 300 mg once daily. Increase at weekly intervals in 300-450 mg/day increments according to response. In ESRD on dialysis, use immediate-release oxcarbazepine instead of OXTELLAR XR. These are adult initiation and formulation instructions, not a pediatric renal table or authorization to substitute immediate-release and ER doses indiscriminately. Take the ER tablets on an empty stomach and swallow whole.
