Renal dosing, restrictions and evidence limits
For renal failure requiring dialysis, start the long-acting intragluteal injection at 10 mg every 4 weeks. Other renal impairment uses the nonrenal starting dose, 20 mg every 4 weeks. Adjust maintenance to response and tolerability. These are starting instructions, not universal maintenance ceilings. Preserve the indication-specific transition: carcinoid/VIPoma patients switching from subcutaneous Sandostatin continue the prior subcutaneous dose for at least 2 weeks; some need 3-4 weeks. Do not apply this depot schedule to oral or short-acting octreotide.
