Renal dosing, restrictions and evidence limits

At GFR 15 to <30 mL/min/1.73 m2, adults start 12.5 mg once daily with a maximum of 25 mg daily. For ages 13-17 with severe impairment, the maximum is 12.5 mg daily if tolerated; do not escalate. MYDAYIS is not recommended in ESRD (GFR <15). These age-specific maximums must not be replaced by another amphetamine formulation's limits.
