Usual dosing and formulation
Inotrope/vasodilator: load 50 micrograms/kg slowly over 10 minutes, followed by continuous infusion 0.375-0.75 micrograms/kg/min.
Use a 1 mg/mL vial for the loading dose. The 200 micrograms/mL premixed flexible container in D5W is for maintenance infusion, not loading. At 1 mg/mL, the loading volumes for weights 30, 40, 50, 60, 70, 80, 90, 100, 110 and 120 kg are 1.5, 2, 2.5, 3, 3.5, 4, 4.5, 5, 5.5 and 6 mL.
The source maintenance table lists minimum 0.375, standard 0.50 and maximum 0.75 micrograms/kg/min, with corresponding listed 24-hour totals 0.59, 0.77 and 1.13 mg/kg. These source totals are preserved without recalculation.
Adjust to hemodynamic/clinical response and monitor closely. Studies described improved cardiac output and lower pulmonary capillary wedge pressure.
Renal Dosing
Creatinine clearance is indexed, mL/min/1.73 m2. Listed infusion rates (micrograms/kg/min): above 50, no change; at 50, 0.43; at 40, 0.38; at 30, 0.33; at 20, 0.28; at 10, 0.23; at 5, 0.20.
