Infuse over 1 hour. Renal impairment does not require adjustment, and no supplemental dose is required after hemodialysis. Monitor renal and hepatic tests despite the absence of a dose reduction. The label has not adequately studied Candida endocarditis, osteomyelitis or meningoencephalitis; the institutional high-dose protocols are separate recommendations.

Historical renal / replacement-therapy guidance

The 20-50, 10-20 and <10 mL/min bands retain the usual indication-specific dose. PD, HD, high-flux and continuous therapy also retain usual doses; the book describes no dialysis removal. Its broad dose range is indication- and weight-dependent, so use the separate U.S. adult indication doses. The book notes reports of renal dysfunction/AKI despite the no-adjustment recommendation.

Dosing / renal protocol and limits

For candidemia, invasive candidiasis and the listed ICU/neutropenic empiric-treatment settings, the adult regimen is 100 mg IV daily. Esophageal candidiasis, endocarditis and cardiac-device infection use 150 mg daily. Candida prevention in selected transplant, neutropenic or hematologic-malignancy patients uses 50-100 mg daily.

UCSF retains these doses in renal dysfunction and its dialysis table. Obesity, elevated MICs and some deep infections may require specialist-directed higher doses. This institutional indication list is broader than some labeled uses.
